New Client Forms

Please know, it is imperative that I have these forms prior to or at our first session. This means that if you do not bring your forms with you to my office for your first visit, you will not be seen. Please let me know if you are having any trouble or have questions regarding these forms well in advance of our scheduled appointment as I will not have these forms available at my office for you. Having all of the housekeeping items ready prior to your arrival maximizes the time we have to work together.

Why Forms Cannot Be Filled Out or Signed Electronically

Please know that due to confidentiality guidelines, these forms are in PDF format.  This means that you have to print them and fill them out.  You will not be able to fill them out or sign them electronically.  Please bring them with you to our first appointment, if you will be seeing me over the phone or would like to email these completed forms to me, please scan them and send them via my safe and encrypted email address To ensure a high level of confidentiality, I encourage you to register for a free, safe and encrypted email address via so we may correspond privately while we work together, as well.
